Helicobacter pylori (H. pylori) is a type of bacterium that can live in the lining of the stomach. H. pylori infection is common, and a positive H. pylori test does not mean that a person has a serious disease or stomach cancer.
What matters is to understand what the test result means, have the test performed under appropriate conditions, and receive appropriate treatment when indicated.

1. WHAT IS THE C14 UREA BREATH TEST?
The C14 Urea Breath Test is a non-invasive test used to detect active H. pylori infection in the stomach.
H. pylori produces an enzyme called urease. When a person drinks a small amount of urea labeled with carbon-14 (C14), the urea will be broken down if H. pylori is present in the stomach, producing CO₂ containing C14. This CO₂ enters the bloodstream, is transported to the lungs, and is then exhaled.
The testing device measures the amount of C14 in the breath sample to determine whether there is evidence of an active H. pylori infection.

2. HOW SHOULD YOU PREPARE BEFORE A C14 UREA BREATH TEST?
This is an important step because certain medications can reduce the number or activity of H. pylori, potentially causing a false-negative result.
In general, before undergoing a urea breath test, patients may need to:
Some other acid-suppressing medications may also affect test results. Therefore, if you are taking medication for a stomach condition or have recently taken antibiotics, inform your doctor before the test so that you can receive appropriate instructions regarding how long you should stop the medication.
Do not stop medications prescribed by your doctor for other medical conditions on your own simply to prepare for the test.
3. WHAT DOES A POSITIVE C14 TEST RESULT MEAN?
A positive result indicates evidence of an active H. pylori infection in the stomach.
H. pylori is a common bacterial infection associated with gastritis and gastric or duodenal ulcers. Over the long term, H. pylori infection may increase the risk of certain stomach conditions, including stomach cancer.
However: H. PYLORI POSITIVE ≠ STOMACH CANCER
Detecting H. pylori allows the patient to be appropriately evaluated and treated, which may help reduce the long-term risks associated with H. pylori infection.
Patients should not self-medicate with antibiotics or reuse a previous H. pylori treatment prescription. The choice of treatment regimen should take into account the patient's history of antibiotic use, previous H. pylori treatment, drug allergies, and individual clinical circumstances.
4. WHAT DOES A NEGATIVE C14 TEST RESULT MEAN?
A negative result means that the test did not detect evidence of an active H. pylori infection at the time the test was performed.
However, it is important to determine whether the test was performed under appropriate conditions.
If the patient has recently used:
the negative result may not be reliable because of the risk of a false-negative result.
In such cases, the doctor may consider repeating the test after the medications have been discontinued for the appropriate period.
5. THE C14 TEST IS NEGATIVE, BUT STOMACH PAIN PERSISTS
This can certainly happen.
Symptoms such as epigastric pain, bloating, indigestion, nausea, belching, or heartburn are not always caused by H. pylori.
Upper gastrointestinal symptoms may be associated with various conditions, including gastroesophageal reflux disease (GERD), functional dyspepsia, gastric or duodenal disorders, or other causes.
Therefore, if the H. pylori test is negative but symptoms persist, further evaluation should focus on other possible causes of the symptoms rather than focusing solely on detecting H. pylori infection.
6. IF H. PYLORI IS DETECTED, WHAT SHOULD YOU DO?
Once an H. pylori infection has been confirmed, the doctor will select an appropriate eradication regimen based on the patient's individual circumstances.
During treatment, patients should:
One important point to remember is that improvement in symptoms does not necessarily mean that H. pylori has been eradicated.
Conversely, having some persistent symptoms after treatment does not necessarily mean that H. pylori treatment has failed.
An appropriate follow-up test is required to confirm whether H. pylori has been successfully eradicated.
7. AFTER H. PYLORI TREATMENT, DO YOU NEED TO BE TESTED AGAIN?

After treatment, follow-up testing is needed to confirm that H. pylori has been successfully eradicated.
In general, testing should be performed at least 4 weeks after completing treatment. PPIs should also be discontinued for at least 2 weeks, and other medications that may affect the test result should be avoided according to your doctor's instructions.
The urea breath test is one of the commonly used methods to confirm H. pylori eradication.
Do not rely solely on improvement in symptoms to determine whether H. pylori has been eradicated.
8. FOUR KEY POINTS TO REMEMBER WHEN HAVING A C14 H. PYLORI TEST
Indicates evidence of an active H. pylori infection, but does not mean that you have stomach cancer.
Make sure the test was performed after stopping medications that may cause a false-negative result for the recommended period.
Pay attention to the recommended medication-free periods: 4 weeks for antibiotics/bismuth and 2 weeks for PPIs.
A follow-up test is needed to confirm H. pylori eradication. Treatment success should not be assessed solely based on changes in symptoms.
H. pylori is a bacterial infection that can be detected and treated. What matters is understanding what the test result means, choosing an appropriate treatment regimen, and having follow-up testing at the right time to confirm H. pylori eradication.
